If you are a recipient of a grant from The Gene and Irene Wockner Foundation, we invite you to use the following form to provide your required Report of Impact statement. Please be sure to upload supporting documents, too.
To be eligible for future grant consideration, please submit your Report of Impact by December 31st of the year in which your organization received funding. While pictures are appreciated, they are not mandatory. Any submitted images may be featured on our website.
